There is sometimes a vertical fold of skin present in the inner corner of the eye (the medial canthus), known as the epicanthic fold. This fold blunts and shortens the inner corner of the eye and causes the eye to look horizontally shorter. In addition to this, the epicanthic fold also creates downward tension on the skin near the inner corner of the eye, resulting in the double eyelid crease to collapse in that area and join the medial canthus.

In the absence of an eyelid crease, eyes can look smaller and less bright, with the eyelashes tending to be angled downwards and inwards, causing them to be less visible. Skin laxity and puffiness tend to also be more noticeable as you age, thus, eyelids without a crease tend to age earlier and faster. All things being equal, having double eyelids tends to make the eyes look bigger, brighter and with more visible eyelashes angled more upwards and outwards. All of this results in having more attractive eyes.

Double Eyelid Surgery

Medial epicanthoplasty, or eyelid surgery,is a surgical procedure involves removing the excess skin fold, resulting in the inner corner of the eye becoming sharper and longer, horizontally and leading to a more attractive looking eye, overall.

Older surgical techniques produced unsightly scars, whereas as the newer medial epicanthoplasty does not. In recent times, new techniques have been developed that effectively correct the epicanthic fold inconspicuous scars.

Double Eyelids by Suture Upper Blepharoplasty (SUB)

Establishing and maintaining the missing connection between the underside of the skin and the levator muscle is essential to produce the upper eyelid crease and is achievable when using sutures (stitches). Sutures are used to connect those two structures along the line of the intended lid crease.

How do surgically created double eyelids become permanent?

Depending on which sutures are inserted will determine the longevity of the surgically created double eyelid. The sutures will eventually cut through the eyelid tissues and cease to maintain the physical connection between skin and levator muscle where they then remain. Fibrous scar tissue would have formed around the sutures by the time this happens, and it these scar tissues that maintain the skin-muscle connection in the long term. It is the patient’s own scar tissue, and not the sutures that determines the permanency of the result, thus, the more scar tissue that is produced, the more likely the crease will be permanent.

How does a surgical result fail?

Since the permanence of the result depends largely on the formation of scar tissue, therefore the crease could become weaker (i.e. shallower) with time and may even eventually disappear if not enough scar tissue is produced. This is considered as a failure.

How do you make sure that enough scar tissue forms?

The presence of more complicated and tightly tied sutures are the key to producing more scar tissue and increasing the success off the procedure.

What are the advantages of SUB?

SUB is quicker to perform than OUB and is less invasive. A permanent, natural looking new crease without the need for invasive surgery and scars is achieved using suture, not to mention, the eyes will open wider and will look brighter and more alert.

Lateral Canthoplasty

This is the correction of blunt and/or short outer corners of the eye by way of lengthening and sharpening the outer corner of the eye. The surgical scars are well hidden along the margin of the upper and lower eyelid above and below the outer corner of the eye as they are with the medial picanthoplasty eyelid surgery.
